If you have an appointment for the issuing of an Animal Health Certificate for travel to the EU, here's a checklist of things to remember :

• The owner travelling with the pet must attend the appointment - a declaration needs to be signed by the owner at the appointment. If the owner is nominating someone else to travel with the pet please contact us immediately for further guidance.
• You must bring the pet ! We have to read the microchip and carry out a brief health check.
• Bring your rabies vaccination certificate . If this was not issued by Ashwood, please would you upload  a photograph of the certificate showing the microchip and rabies vaccination details below (or email it to ashwoodvets@yahoo.co.uk) so that we can check everything is correct - this helps to avoid delay and/or potential disruption to your travel plans.
• For appointments in the morning, your certificate will usually be available for collection after 4pm that day. For appointments in the afternoon, collection will be the following day onwards. If this is unsuitable, we can sometimes offer a fast-track service at additional cost but this is dependent on clinical workload at the practice.
• Please complete the form below at least 48 hours before your appointment.
